School and Camp Forms:

Please note there will be a $10 fee for those requesting forms without having completed an appointment within the last 1 year.

Patient Forms and Portal:

Intake Forms can be completed online through our portal:

• You will receive an email with a link when you schedule a new appointment.
• Create an account with a username and password. For minors, use the minor's name but a PARENT's date of birth to create the account.
• On the portal website patientally.com press Intake Documents at the TOP of the screen to access and complete forms online. (Disregard the message about missing documents).
• Do not be alarmed if an appointment does not show in the portal; it is a glitch in the system (working on it).
• For Dr. Jani to see any documents you upload, you MUST ATTACH the DOCUMENTS to a message you send to her.

Office Policy for Missed Appointments

Please call to reschedule or cancel an appointment within 24 hours of the scheduled appointment. For missed appointments, a $75 fee will be charged.
